I got the same message from United Health Care, after cheking this website i called the customer service number on my card, the representitive checked my records and said the call was legit
As mentioned earlier, this number is supposedly used by UHC for survey purposes, although they are based in Texas; I've already spoken with them, and they will now mail me the survey instead.
