After contacting the CDC, they provided me with the number 1-877-220-4805 to call, and confirmed that (312) 578-7017 is a legitimate NORC U Chicago phone number, and the immunization survey is legitimate, and fortunately, the representative at the 877 number removed my number from the call list.
After contacting the CDC, I was directed to call 1-877-220-4805 at NORC U Chicago, where I filed a complaint, and my number is supposedly being removed from their call list.
Multiple calls have been received from this number, even after being added to a blocked call list.
